Enrollment in Medical Assistance (MA): Eligibility for housing transition services requires that the individual is a recipient of Medical Assistance.
Age Requirement: Individuals must be 18 years old or older to access these services.
Documented Disability or Condition: A documented disability or disabling condition is necessary.
Experiencing Housing Instability: The individual must be facing challenges related to housing instability.
Services Provided through Housing Transition:
Customized Housing Search: Offering a tailored approach to the housing search, considering the client’s specific needs, preferences, and financial situation to find suitable housing options.
Application Process Assistance: Guiding clients through the housing application process, including support for those with poor rental history, to increase their chances of securing housing.
